Personal and Community Health
Welcome to Personal and Community Health offered online through Illinois Valley Community College Distance Learning. I am your instructor, Julie Sherbeyn, and am very excited for this opportunity to increase your health awareness through this innovative medium. The goal of this course is to encourage you to consider your personal health behaviors relative to current health information. You will have the opportunity to evaluate your current lifestyle and identify alternative options to improve your overall wellness. This an extremely fast-paced eight-week course with no classroom meetings; therefore, it is imperative you are aware of the course requirements and corresponding timeline. This course is totally asynchronous (on your own) to allow for flexibility in your personal schedule while earning college credit.

The course consists of utilizing the text, taking chapter quizzes and unit exams, and completing assigned labs. (Please see the Course Timeline for an outline of course content and corresponding deadlines.) You are required to contact the instructor at the beginning of the course to confirm expectations (please refer to Instructor Contact Information). Contact your instructor after you:

  1. Register for the class through IVCC.

  2. Obtain your class materials.

  3. Complete appropriate online orientation.

Please note: Effective Summer 2011, all students will be responsible for checking their IVCC email. All electronic college correspondence will only be sent to the IVCC email. For information on accessing this account, go to You must include your full name and the course number in which you are enrolled (HPE 1003-100 or HPE 1003-101) in the subject line of all email contact with your instructor. You will not receive any response and your concerns will NOT be addressed if the appropriate format is not followed.
Required Materials:

The textbook is required for this course and is available in the IVCC bookstore.

  • Text: Teague, Mackenzie, & Rosenthal. Your Health Today, Brief 3rd ed. New York, NY: McGraw Hill, 2012.

  • Internet:  Access to Blackboard ( to access all course materials, assessments, labs and your grades.

  • Internet Browser: You must download Respondus Lock Down Browser and utilize it while taking all quizzes and exams. A free download and instructions can be found online in Blackboard in the Course Menu.

Blackboard Usage Instructions:

The course syllabus and timeline, all quizzes and exams, labs and grades can be accessed on Blackboard. Any browser is suitable to view grades, announcements and course documents, but to eliminate technical issues, you must use Respondus as your browser for all quizzes and exams. Blackboard will automatically load when Respondus is launched. All open programs will be suspended, and you will not be able to access anything other than Blackboard until you have completed the assessment. Quizzes and exams will NOT be accessible after the respective deadline (see Course Timeline).


After comprehensive study of each chapter, you will complete a ten-point multiple choice quiz. The quizzes are located in Blackboard under “Course Documents” and are available beginning Tuesday, May 31.

  • There are deadlines for completing each quiz.

    • You will not be awarded a grade if you fail to meet the mandatory timeline (see the Course Timeline).

    • You can work ahead, so plan according to your schedule to complete assessments before deadlines and avoid receiving zeros.

  • There will be absolutely no make-ups allowed on quizzes.

    • Technical issues are not an excuse for failure to complete an assessment; therefore, it is imperative you utilize Respondus and have a reliable online connection and computer.

    • You will not be able to access quizzes once the deadline has passed.

  • You will have fifteen minutes to complete each quiz.

    • Quizzes will be scored and grades will be posted immediately following successful submission.

    • The time limit will require you to have a firm grasp of the information prior to attempting the quiz.

  • You may attempt the quizzes for Chapters 1 through 5 as many times as you wish prior to their respective deadlines.

    • This will allow an opportunity for you to become familiar with online testing and will enable you to maximize your points for those quizzes.

    • You only have one opportunity to complete the quizzes for Chapters 6 through 15.

No extensions or make-ups will be allowed for quizzes, however, two of your lowest quiz scores will be dropped at the end of the course.

After comprehensive study of each unit, you will complete a forty-point exam. Respondus will be used to access all exams located in Blackboard under “Course Documents” available beginning Tuesday, May 31.

  • There are five, forty-point tests.

    • Exams consist of multiple choice, short answer, and essay questions.

    • The tests cover the textbook material from the designated chapters; therefore, I suggest taking notes while thoroughly studying the material.

    • Each test is of equal weight in determining your final grade. The tests are moderately difficult college-level exams.

  • There will be absolutely no make-ups allowed on exams.

    • Technical issues are not an excuse for failure to complete an assessment; therefore, it is imperative you utilize Respondus and have a reliable online connection and computer.

    • You will not be awarded a grade if you fail to meet the mandatory timeline (see the Course Timeline). You will not be able to access exams once the deadline has passed.

    • You can work ahead, so plan according to your schedule to complete assessments before deadlines and avoid receiving zeros.

  • You will have fifty-minutes to complete each exam.

    • The time limit will require you to have a firm grasp of the information prior to attempting exams.

    • Your score will be available after the instructor has had adequate opportunity to review your exam.

No extensions or make-ups will be allowed. You must abide by the deadlines listed in the Course Timeline to avoid receiving a zero for the exam.

Lab Assignments

You have five assigned labs found in the “Personal Health Portfolio” located in the back of your text or in Blackboard under “Lab Assignments” in the Course Menu. Be sure the lab titles and chapter and appendix page numbers correspond to those assigned below. All labs are due in the instructor’s office on or before Friday, July 15. Labs can be mailed or dropped off at the instructor’s office or dropped off at the IVCC information desk or mailroom with instructions to forward to Julie Sherbeyn. Please see instructor contact information included in the Course Assistance section of this document for address and office information. Plan accordingly for labs arriving by July 15 as labs not turned in by the deadline will not be awarded a grade.

Your grade will be determined according to the total number of points you receive on the required assignments, quizzes, and exams. There is a maximum of 400 points.

Grading/Potential Points: Total Points/Grade Scale

Quizzes (15 @ 10 pts. ea.) 150 pts 400 - 358/(100-90%) = A

Class labs (5 @ 10 pts. ea.) 50 pts 357 – 318/(89-80%) = B

Exams (5 @ 40 pts. ea.) 200 pts 317 – 278/(79-70%) = C

Total pts 400 pts 277 – 238/(69-60%) = D

237 - 0/(59-0%) = F

Course Policies:

Class Attendance:

This course is totally online; therefore, when you log into the class you are in attendance. No grades will be awarded for late assignments, so you may want to complete assessments early to avoid last minute emergencies including possible technology failures.

Withdrawal Policy:

  • If you find you are unable to continue this class, YOU must contact the instructor to complete the necessary paper work.

· If you stop participating in the class and don't officially withdraw, you may receive an "F" at the end of the semester.

  • Last Day for Withdrawal : 7/12/11

  • The instructor reserves the right to withdraw students who are not making progress in the course. This will include students who are not meeting time requirements for the course.

Academic Concerns:

If you are a student with a documented cognitive, physical or psychiatric disability (anxiety, depression, bipolar disorder, AD/HD, post-traumatic stress, and others) you may be eligible for academic support services such as extended test time, texts on disc, note taking services, etc... Academic Dishonesty:

Any student caught cheating will receive a zero for the activity in question and may be subject to the student code of conduct for further disciplinary action.

Ensuring Success:

Success in this course depends, primarily, on a disciplined schedule of studying the text and completing the required labs, quizzes and tests according to the designated timeline. Notice that multiple assessments may be due on one day. I suggest you are active in the class daily to keep up with all required timelines and grading opportunities. Again, if any assigned deadline is missed, no points will be awarded for that grading opportunity.
